Welcome to Carrfield Medical Centre. By registering and utilising our services, you agree to follow these Terms and Conditions ("Terms"). If you disagree with any part of these Terms, kindly refrain from using our services. Note that these Terms can change periodically; we encourage you to review them frequently.
We provide a comprehensive array of services as outlined in the General Medical Services (GMS) contract. This includes preventative care, general medical advice, treatment for both acute and chronic conditions, health education, and referrals to secondary care services.
In addition to the services covered under the NHS, we also offer certain non-NHS services that are not covered by the NHS. Please be aware that non-NHS services might be subject to additional charges.
We welcome new patients to our practice. Anyone in our practice area can register with us regardless of race, gender, social class, age, religion, sexual orientation, appearance, disability, or medical condition. We may only decline registration if you live outside of the South Yorkshire Integrated Care Board area, if we are not currently accepting new patients due to capacity, or if we do not offer the services you require. Please note that registration with our practice may be subject to proof of identification and address.
You can schedule appointments online, via phone, or in-person at our surgery. If you're unable to attend a scheduled appointment, we request a minimum of 24 hours' notice for cancellation. This enables us to offer the slot to another patient.
We issue prescriptions according to NHS guidelines and the clinical judgement of our GP or healthcare professional. If you require repeat prescriptions, please place your request at least 48 hours in advance. We provide an electronic prescription service for your convenience, allowing you to collect your medications from a chosen pharmacy.
We are devoted to preserving the privacy and confidentiality of your personal and health information in line with the General Data Protection Regulation (GDPR) and the NHS Code of Practice on Confidentiality. Please read our privacy notice for more information.
We expect our patients to treat our staff and other patients with respect and courtesy. We have a zero-tolerance stance towards any form of abuse, discrimination, or violence. Breaching this policy may result in removal from our patient list and potential legal consequences.
We reserve the right to amend these Terms as needed. While we'll attempt to inform you of significant modifications, it remains your responsibility to review these Terms consistently.
We are committed to delivering exceptional healthcare to all our patients. If you have a complaint or suggestion, please reach out to us. We'll address your complaint in accordance with our complaints procedure and NHS guidelines.
These Terms, along with any dispute or claim arising from or connected to them, are governed by the laws of England and Wales. You consent to the exclusive jurisdiction of the courts of England and Wales for resolution of any dispute or claim.
